Page 20 of Lakeland Community College General Information by Lakeland Community CollegeAcademic Standards and Policies 21 Withdrawal The notation of gWh will be made on the transcript for the student who withdraws from the beginning of the second week through the end of the ninth week during fall semester and the end of the tenth week during spring semester; the end of the third week during the five-week summer session; and the end of the fifth week the eight-week summer session. See the class schedule for exact dates Not Available The notation of gNAh will be made to indicate that no grade is available from the instructor. When the instructor submits the grade, the gNAh grade will be changed to the appropriate letter grade Repeating Coursework The last grade received for a repeated course will thereafter be substituted for the former grade in calculating the studentfs cumulative point average. (The original grade will still be shown on the transcript.) Repeated courses are treated in different ways by colleges to which a student might transfer. The student should, therefore, check with these colleges if he or she has repeated courses to understand fully how they will be treated on the transcript. Grade Reports A grade report will be available via the web at www.lakelandcc.edu/ LOL or can be mailed at studentfs request Grade Appeals A studentfs request for a grade change will be considered only during the term immediately following the one in which the grade was assigned, an exception being that grades assigned during the spring semester may be changed during either the following summer or fall term Academic Forgiveness The Academic Forgiveness Policy pertains to students who were enrolled in one program or degree and are now enrolled in a different program or degree. This policy provides students with the opportunity to have their accumulative grade point average reflect their academic success in their current program or degree This option is a one-time irreversibleAcademic Forgiveness Policy which allows previously earned credit, not applicable to the current program or degree, to be deleted from the transcript. Students must ascertain if the academic forgiveness policy would affect their current status, particularly financial aid eligibility Students who have completed and earned a degree from Lakeland are not eligible for this policy A student who has changed his/her degree or program objective may petition the Associate Provost for Student and Learning Support Services for academic forgiveness The student needs to do the following: 1 Discuss the change with a counselor to determine if the academic forgiveness is an appropriate option 2 In order to request academic forgiveness, the student must demonstrate a clear change in degree or program objective. Completed courses usually reflect the change 3 The counselor will review the studentfs academic record to determine which courses completed to date are applicable to the studentfs new academic objective 4 If deemed appropriate, the counselor will recommend that credit be deleted for courses already taken that are not part of the new academic objective 5 The student must have completed at least 12 credits with a 2.0 GPA in the new degree or program. These 12 credits must have been earned beginning with the semester in which the change occurred and any subsequent semesters. Credits earned prior to the change may still count toward the new degree or program requirements but are not considered as part of the 12 credits needed for the petition to proceed 6 The Petition for Academic Forgiveness is to be completed only after the 12 credits needed have been successfully recorded on the studentfs transcript 7 The student must be enrolled in classes at Lakeland Community College the semester in which the petition is fi 8 The recommended Academic Forgiveness is then sent to the Associate Provost for Student and Learning Support Services for review 9 The Associate Provost for Student and Learning Support Services will forward the recommended Petition for Academic Forgiveness to the program chair or dean of the studentfs new academic program for review 10 Upon approval, a revised transcript will be created 11 This procedure for academic forgiveness is a one-time option and is irreversible 12 If a student disagrees with the counselor evaluation, he/she may appeal directly to the Associate Provost for Student and Learning Support Services Academic Standards Academic Probation 1 A student will be placed on Academic Probation after attempting a minimum of 12 semester credits and both the semester GPA and cumulative GPA fall below 2.0 2 A student on Academic Probation is required to: Complete a Plan for Academic Student Success (PASS) form prior to enrolling in the first semester of probation 3 A student on academic probation may attempt a maximum of 18 additional semester credits. The student will be removed from academic probation during the 18 credit hour period when his/her cumulative GPA is 2.0 or higher.
